Pediatric Care Management consists of a Social Work Care Manager and two MA (medical assistant) Health Coaches. This team offers support to parents and providers to ensure healthy outcomes for children. Many families experience barriers around accessing services and Care Management works with them to identify those barriers and overcome them.
Care Management oversees the daily operations of the Key Clinic, a large foster care clinic, and the CHAMP Clinic, an outpatient clinic for infants in need of pharmacological intervention to wean from opiate exposure in utero.
Care Management also provides Behavioral Health Home services to children in need of case management services.
